與外國銀行 切結書之書寫. - 銀行

我與國外銀行 想進行 關戶帳戶的動作 是利用fax和e-mail的方試進行

但在 切結書的最後 出現了一行文字 (之前的文書 都是用列點式的說明)

這行文字為



Dated at______(1)__________this ___(2)____day of ____(3)_____20___(4)___


我現在 只知道 現在第(4)格的 答案是2009

不知 板上的板友 可否為我解惑 (1)(2)(3) 其它的選項呢?

另外 我還有另一個疑問

在Executed as a deed by the Customer:中

為啥 相同的東西 會有兩排?

ex:

Executed as a deed by the Customer:

____________ ____________

Individual/Director/Trustee Individual/Director/Trustee

___________ ____________
Full Name Full Name
