病歷註記但實際沒有的疾病是否需告知 - 保險

大家好
這兩天趁著週末跟朋友一起看了一些保險的資訊，我自己有找了一些保險公司的健康告知書來看，並自己核對健康存摺的病歷註記，結果發現其中一項健康告知書上有列出一年內是否曾患有的疾病，我在健康存摺今年的病歷紀錄中也有寫出此診斷，但是我非常確定我並沒有這個疾病，當時看診時也非常確定當下醫生是說並非此病，Google後看到一些案例是說醫生有時候為了開藥所以需要下某種診斷，但是實際上病患不一定有患病。
主要是想請問，如果是這個狀況的話，那我的健康告知書上到底是要勾是還是否呢？因為病歷上有出現這個病名，但我確定自己是沒有這個疾病，我擔心勾否核保後未來會被保險公司說不實告知而解約或拒絕理賠，但我實際又沒有患病，勾是也很冤枉，假如想保的險種又有限制標準體承保，那有這個紀錄應該要怎麼辦，不知道怎麼做比較好，還請各位指點，非常感謝
