1: A copy of the credit card statement stating your billing address.
2: A copy of front and back of the credit card. (for security purposes
please black out the last 3 digits on the back of the card in the signature
area).

The copies can be faxed to us at 00000000000 or it can be sent by email in
JPEG format.
If we do not hear from you within 5 days, the order will be automatically
cancelled.
If you would like to pay by money order, please e-mail us back
to let us know so we will not cancel your order.
